Position Summary: Intake Care Managers serve as the initial in-person contact with applicants who are newly referred to the State Home Care Program. They are responsible to conduct in-home initial assessments of these applicants living in the community to determine eligibility for the State Home Care Program and assist them in obtaining services and programs which will enhance their quality of life and allow them to live as independently as possible with dignity and respect.


Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)

  • Manage assignment of eight (8) new Home Care Program referrals per week.
  • Conduct in-home visits to assess older adults' physical, social, emotional, and environmental status to evaluate their home care service needs and determine eligibility for the State Home Care Program in accordance with Executive Office of Elder Affairs regulations.
  • Develop and implement a service plan to address unmet needs by making referrals to contracted service providers and other community agencies.
  • Make referrals to community resources as needed to address needs should the applicant not qualify for or refuse state funded services, or to supplement State Home Care services.
  • Provide advocacy and assistance for consumers in securing benefits and services when consumers or their informal supports are unable to do so.
  • Collaborate with other agency departments and outside agencies as needed to address consumer needs.
  • Arrange for crisis intervention services when necessary.
  • Complete and maintain up-to-date documentation and data entry, including case files and statistical records, as required by the agency and Executive Office of Elder Affairs.
  • Work collaboratively with manager and peers to cover the workload as needed during times of low staffing due to vacancies, leaves and vacations.
  • Comply with regulations governing Aging Services Access Points as set forth by the Executive Office of Elder Affairs.
  • Other duties as assigned.

Participation in Staff Development Programs:

  • Participate in peer staff meetings and supervision.
  • Attend and participate in Home Care and general staff meetings as scheduled.
  • Participate in in-service training, courses, and workshops to maintain professional expertise and develop additional skills.
  • Assist in the orientation and training of new staff members as needed.

Qualifications: Bachelors degree in social work, human services or a related field required. Associates degree with significant relevant work experience may be substituted for a portion of degree. A minimum of 1 year of experience working with elders and conducting in home assessments strongly preferred. Strong written, verbal and computer skills as well as excellent time management and organizational skills and the ability to work successfully within a fast-paced environment required. Valid Driver's License and reliable transportation required as field work is an essential component of the position. Bilingual capacity in Spanish is preferred.


Hours: 37.5 per week
