AgeWell PACE is a non-profit established by Santa Rosa Community Health and LifeLong Medical Care in 2021 to develop and operate a Program of All-Inclusive Care for the Elderly (PACE) to serve Marin and Sonoma County residents. As community health centers with aligned priorities and complementary resources, SRCH and LifeLong will share leadership on the AgeWell PACE Board of Directors.
Opening in July 2023, AgeWell PACE will provide medical care, case management, transportation, and adult day services to 300 low-income, medically complex older adults living in Marin and Sonoma County at our newly renovated facility in Rohnert Park. We expect that there will be about 60 employees by the end of the second year of operation.
ABOUT AgeWell PACE Partners:
Santa Rosa Community Health: For 26 years, SRCH has led the health care field in Sonoma County, consistently evolving and growing our services and programs to meet the needs of our community. Starting from a single health center, we now provide high quality, culturally responsive medical, dental, and mental health care for over 42,000 people. More than 7,300 of SRCH patients are age 55 , a subset of whom will be eligible for PACE services.
LifeLong Medical Care: LifeLong has a 45-year history of prioritizing care for older adults, beginning with the Gray Panthers who opened LifeLong’s first health center in 1976 (the Over 60 Health Center in Berkeley). Among FQHCs nationally, LifeLong is a leader in developing models of care for the elderly. As a community health center serving 60,000 people annually (primarily in the East Bay) and the only provider of Adult Day Health Care services in Marin/Sonoma, LifeLong is well-equipped to partner with SRCH to grow AgeWell PACE.
ABOUT the Primary Care Physician position:
The PACE Primary Care Physician is responsible for providing direct primary medical care to participants of the PACE program and to supervise medical care personnel, such as Nurse Practitioners, RNs, and Medical Assistants. The PACE Primary Care Physician provides care to PACE participants in the context of the Interdisciplinary Team (IDT).
Position Responsibilities:
- Perform comprehensive history and physicals on new referrals.
- Perform interval history and physicals on established participants, completed quarterly or as indicated.
- Evaluate and treat participants with episodic illnesses.
- Refer participants to Medical Specialist(s) as indicated.
- Admit participants to the hospital when indicated: (a) Provide primary care (attending) responsibilities for management; (b) Provide updates to the IDT on a regular basis; (c) Judiciously utilize specialty consultants; (d) Responsible for coordinating discharge planning with the IDT.
- Manage all care of participants who reside in nursing homes: (a) Provide regular visits; (b) Perform telephone contacts with nursing home staff as required; (c) Admit nursing home participants to hospital when necessary.
- Actively participate in family conferences regarding care planning and implementation.
- Coordinate 24- hour care delivery.
- Document participant changes appropriately in the medical records and communicate participant changes to IDT in a timely manner.
- Participate as a member of the IDT. Attend staff meetings and take part in participant care planning.
- Maintain and increase education regarding quality/clinical standards and best practices for the geriatric population.
- Participate in the Ethics and Quality Improvement Committees and/or work groups as assigned.
Supervisory Duties:
Team member(s) this position supervises: advanced practice providers as needed.

- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Experience & Education:
- Valid MD license in good standing to practice medicine in the State of California.
- Graduate of an accredited School of Medicine.
- Minimum two years clinical experience required.
- Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from AgeWell PACE on working with a frail or elderly population upon hiring.
- Fellowship or additional training in Geriatrics preferred.
- Bilingual in English and Spanish, a plus

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. - Physical demands: Team member will be seeing elderly patients in a busy clinic environment. Team member must be able to stand, bend, walk and sit for extended periods of time, as well as work at the computer for long periods. Team member is required to use hands and fingers, especially for typing on the computer and using the mouse. Team member must be able to talk and hear, particularly for regular communication on the phone and with patients.
- Work Environment: Team member must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE members’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled office temperatures.
