Job description
Aging & Adult Care of Central Washington is looking for a Case Manager to join the Moses Lake team. This position is responsible for case management services for older adults and/or persons with disabilities in need of community based long term care services. Case Managers perform comprehensive functional assessments, develop care plans, coordinate providers and maintain client records.
Education and Other Requirements:
A Bachelor's Degree in Social Work, Social Science or related field and two years of paid social service experience (please see job description for more details). Bilingual English/Spanish preferred.
Must have reliable vehicle, valid WA State Drivers license, and minimum state required vehicle insurance. A criminal background check will be completed.
