Overview:
We are seeking a compassionate and dedicated Care Specialist to join our team. Performs the assessment, development, implementation, authorization, processing and evaluation of community based services for the elderly.
Examples of Work Performed:
Process and coordinate community based services for the elderly.
Authorize providers and services as needed.
Maintenance of records of the clients.
Serve as an advocate for the clients.
Qualifications:
A Baccalaureate degree in Human Services
