WHO YOU'LL WORK WITH (AND WHAT THEY HAVE TO SAY)

The Homecare Practice Manager in collaboration with other members of Homecare leadership will educate and provide guidance to clinical staff to ensure quality and service expectations.

HOW YOU’LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)

The Homecare Practice Manager is responsible for all aspects of care delivery within their team to ensure quality, compliance, efficiency, consistency and continuity of care. The role is responsible for investigating and responding to patient events/concerns, auditing for quality, compliance, timeliness and consistent practices, and fostering and growing facility relationships.

  • Ensuring Quality and Consistency of Home Hospice Services
    • Conduct audits, supervisory visits, and collaborate with others to ensure clinical staff provide quality, consistent, and efficient hospice services while maintaining regulatory compliance and practicing within the scope of their discipline.
    • Assist in communicating expectations and accountability for established standards, adherence to clinical policies, procedures, and compliance with state and federal regulations.
    • Facilitate bi-weekly (minimum) interdisciplinary team meetings to promote effective review of the patient plan of care.
    • Responsible for follow up on all Homecare events entered in Agrace’s Incident Management System. Establish corrective action plans in collaboration with stakeholders. Participate in investigations related to patient incidents as required.
    • Foster and grow facility relationships and respond to facility concerns/opportunities in collaboration with Access team.
  • Quality and Service
    • Assist in leading of continuous improvement efforts for all homecare clinical operations.
    • Collaborate with Quality and Clinical Education teams to create a culture of quality. Drive quality improvement and Quality Assurance Performance Improvement (QAPI) measures.
    • Participate in QAPI meetings, workgroups and initiatives.
    • Proactively work with Team Managers to address patient/family/partner concerns.
    • Analyze satisfaction survey data and collaborate with stakeholders to develop, implement, and audit action plans to address key drivers of patient, family, and partner satisfaction.
  • Financial Performance
    • Monitor team financial performance, specifically about medication utilization and concurrent services. Facilitate ongoing follow up with the interdisciplinary team to ensure interventions remain appropriate with the patient’s plan of care.
  • Management of Staff and Employee Engagement
    • Develop and direct engagement plans to ensure high levels of team engagement in collaboration with team managers and engagement.
  • Other Duties
    • Participate in internal work groups, meetings and committees relative to position responsibilities.

YOU BRING (EXPERIENCE & QUALIFICATIONS)

  • Bachelor’s degree with clinical background, required; Master’s preferred, or equivalent experience
  • Current, unrestricted Wisconsin RN license or SW license
  • Hospice and Palliative Care certification within one year of employment
  • 5 - 7 years clinical leadership and/or a healthcare administrative role, required; hospice environment, preferred
  • Five years progressively responsible supervisory experience
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Knowledge of and experience with health care delivery systems, policies, accreditation standards, laws, regulations and reimbursement practices required; hospice-specific knowledge and experience, preferred
  • Experience conceiving, designing, developing, executing and maintaining project plans and administrative policies and procedures for compliance with regulations
  • Ability to review, collect, summarize and analyze complex data
  • Ability to communicate financial/business/technical/clinical information to all levels within and outside the organization, including group presentation skills
  • Experience in project management, strategic planning and execution, budgeting, and controlling costs
  • Intermediate Microsoft Office Suite skills
  • Ability to work effectively and cooperatively with others
  • Experience using electronic health record software
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics and standard precautions
  • Ability to be flexible in time and task to meet organizational needs
