INTRODUCTION:

The purpose of this position is to assist in the planning, developing, organizing, implementing, evaluating and directing of a meaningful activity programs in compliance with current federal, state and local standards and in accordance with the established policies and procedures of the facility.


ESSENTIAL JOB DUTIES:

To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.


  • Coordinate group and individual activities for the benefit of the patients, with emphasis on their interests and needs
  • Collaborate with volunteers, families, community agencies, etc. Fundraisers may also be planned
  • Recognize and respond to changes in patients’ conditions and document observations, interventions and outcomes
  • Access and evaluate residents’ individual preferences; plan meaningful activities accordingly
  • Plan activities and establish calendar; ensure posting is accessible to facility members and staff
  • Establish and maintain community involvement and create a home-like atmosphere for patients
  • Participate in care plan meetings
  • Organize, develop, implement and distribute newsletter and news articles for local newspaper(s)
  • Perform emergency procedures such as cardiopulmonary resuscitation (CPR)
  • Assist with admitting, transferring and discharging patients
  • Develop, maintain and periodically update written policies and procedures that govern the functions of the Activities Department
  • Participate in surveys made by authorized government agencies
  • Facilitate the coordination of the Activities Department services and other departments to maintain quality care for patients
  • Review, monitor, intervene and document complaints and grievances from patients, families, visitors and employees
  • Prepare reports and recommendations for the Administrator regarding the operation of the Activities Department
  • Review and revise activity care plans on a regular basis within the guidelines of state and federal regulations and facility policies
  • Maintain privacy and confidentiality of records, conditions, and other information relating to patients, employees, and facility
  • Follow the established universal precautions and isolation procedures
  • Encourage an atmosphere of optimism, warmth and interest in patients’ personal and health care needs

JOB REQUIREMENTS:

  • Punctuality, reliability
  • Proficient interpersonal skills
  • Professional communication skills, both written and verbal
  • Successful completion of required training
  • Position may require flexible hours, unscheduled overtime or occasional week-end work
  • Some travel may be required

Required Computer Software/Equipment used:

  • Ability to learn HRIS and/or scheduling systems

REQUIRED QUALIFICATIONS:

  • Education:

o High school diploma or equivalent is required

o Prefer Associates or higher

  • Experience:

o Minimum of six (6) months’ experience working in a skilled nursing facility preferred

o Healthcare experience strongly preferred

  • License/Certification:

o CPR

SCOPE AND LEVEL OF WORK:

  • Complexity: Duties assigned are not generally complex. Work assignment is performed within an established framework under general instructions.

  • Communications: Contacts with the public or employees where explanatory or interpretive information is exchanged, defended, and gathered; discretion and judgment are required within the parameters of the job function.
  • Supervision Received: Performs work with supervisory oversight. Under general supervision, receives assignments and is expected to carry them through to completion with substantial independence. Work is reviewed for adherence to instructions, accuracy, completeness, and conformance to standard practice or precedent. Recurring work clearly covered by guidelines may or may not be reviewed.

SUPERVISORY RESPONSIBILITES:

  • Does not have supervisory responsibilities
  • Supervisor frequently determines priorities
  • Occasionally needs manager’s direction due to extraordinary circumstances
