Job Overview:

As a Case Manager, you will be responsible for providing comprehensive care and support to individuals with intellectual disabilities. This position requires strong interpersonal skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities:
- Conduct assessments to determine the needs of clients and develop individualized care plans
- Coordinate and monitor services provided to clients, ensuring their needs are met
- Advocate for clients' rights and ensure they receive appropriate care
- Collaborate with healthcare professionals, and other stakeholders to ensure continuity of care
- Provide counseling and emotional support to clients and their families
- Monitor client progress and adjust care plans as necessary
- Maintain accurate and up-to-date documentation of client interactions and services provided
- Hire, Fire, and Supervise the House Managers/Direct Care staff in meal preparation, medication management, and other daily activities as needed
Experience:
- Previous experience in ICF group homes preferred or - Social work or a related field
- Strong organizational skills and the ability to prioritize tasks effectively
- Excellent communication skills, both written and verbal
- Ability to handle conflict situations with empathy and professionalism
- Proficiency in computer applications for documentation purposes
