The right candidate will facilitate care for adult clients living with substance use disorder by assessing their needs, evaluating treatment options, coordinating care and gauging progress.
Essential Duties & Responsibilities:
- Provide case management to clients, assisting clients in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need, and including coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals.
- Accomplish clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches.
- Monitor cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintain client records in a timely, orderly manner according to the guidelines established in the agency treatment manual, reviewing case notes; logging events and progress, and documenting all billable interactions into AKAIMS, utilizing DAP note formatting.
- Perform intake assessment interviews using the bio-psychosocial assessment documents, the DSM-5 Diagnostic Criteria and the ASAM: reviewing counselor evaluations, treatment objectives, and plans.
- Monitor cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Communicate clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to clinical team and family; identifying treatment influences.
- Work collaboratively with counselors, clients, families, physicians, and nurse to ensure high quality care.
- Prepare for clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources, and assisting them in getting funding for housing, transportation, education, and other needed items.
- Work with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs.
- Write crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer support to accomplish immediate/crisis goals.
- Establish treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation, using the agency vehicle, and escort.
- Help clients understand and employ recovery strategies.
- Navigate multiple grants and organization programs.
- Prepare for and participates in weekly client case reviews, by collecting, analyzing, and summarizing treatment and results data and trends, compiling statistics.
- Coordinate and facilitate client care through assessment, evaluation, planning, and implementation.
- Assist clients with continuing care planning and referrals by working with clients, family and friend support networks, health care professionals, and partnering agencies and referral sources, ensuring the clients receives the best treatment and case management options based upon the client’s needs.
- Supervise client in social/recreational activities.
- Facilitate psycho-educational and process groups, improving staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
- Manage discharge plans upon completion of treatment and follow up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health.
- Adhere to State and Federal confidentiality laws.
- Attend and participate in the weekly AARS Meeting.
- Provide documents and data for agency grant compliance reports.
- Participate in staff meetings and training sessions.
- Learn and maintain confidentiality of client information within Federal and State guidelines.
- Participate in evaluation of job performance.
- Complete RELIAS training course and other training as assigned.
- Maintain project logs, reports and records in appropriate files and database(s).
- Participate in staff training and conduct training sessions as requested.
- Prepare and submit timely reports and records to the Clinical Manager.
- Prepare and submit incidents reports as needed both positive and negative to Clinical Manager and other staff.
- Complete other duties as assigned.
Minimum Qualifications & Required Skills/Abilities:
- Have High School Diploma or GED, an associate degree in human services or related field is preferred. An equivalent combination of education and experience may be accepted.
- Acquire Chemical Dependency Counselor II or QAP equivalent certification within 3 years of hire date.
- Prefer some experience working in a residential setting.
- Must be knowledgeable of the interaction between chemical dependency and mental illness and the treatment issues related to dual disorders.
- Is culturally sensitive and aware of the State's cultural diversities.
- Is able to supervise and provide leadership to clients in a variety of settings.
- Is able to communicate effectively verbally and in written form.
- Ability to make decisions and exercise good judgment.
- Ability to work independently with minimal supervision.
- Have strong public speaking and organizational skills.
- Have strong problem-solving skills.
- Observes participants rights to confidentiality.
- Ability to coordinate multiple activities.
- Is capable of maintaining client records.
- Is capable of maintaining client confidentiality as per State and Federal regulations.
Required:
- Has a current TB Test (obtainable within the first 30 days of employment).
- Has a current First Aid/CPR Cards (obtainable within the first 90 days of employment) Valid Alaska Driver's License and is insurable.
- Has appropriate computer skills.
- Has to pass a mandated criminal background check.
- Has been vaccinated for COVID.
- Is a role model positive behavior - enforce client rules.
