ALC Primary Care @ Home has an excellent Clinical Team who treats elderly and other adults suffering from an illness, injury, chronic conditions or have complex health issues in the safety, privacy and comfort of their home. Compensation is based on position preference (full-time, part-time, contracted)
- Certified Nurse Practitioner
- Minimum of 2 years of Nurse Practitioner experience
- Active DEA license or seeking a sponsor to obtain DEA license
We are growing and seeking a Visiting Nurse Practitioner to share the same passion for excellence to deliver high quality and compassionate primary care medicine.
The Visiting Nurse Practitioner delivers primary health care and patient care management in a residential setting.

- Teamed with a medical assistant and patient care coordinator
- Essential Duties and Responsibilities
- Takes history, examines, determines diagnoses, and treats scheduled patients
- Provides written documentation of patient's visit, per NCQA standards
- Takes patient vital signs, as necessary, if Medical Assistant is unable to obtain
- Instructs patients/caregivers on proper medication and treatment regimens
- Works with Patient Care Coordinator to assure orders and referrals are properly/timely placed
- Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis and treatment
- Maintains patient confidentiality
- Attends required meetings and in-services and participates in committees, as requested
- Participates in professional development activities and maintains professional licenses and affiliations
- Performs related work as required
REQUIRED Knowledge, Skills, and Experience
- Certified Nurse Practitioner
- Master's (Preferred)
- RN (Preferred)
- Minimum of 2 years of Nurse Practitioner experience
- Active DEA license or we can help you obtain license
- Preferred experience in Geriatrics or complex patients
- Preferred EMR experience'
- Visiting NP will be 'On the road'
