Job Summary:  Assesses, plans and implements the nursing care of residents within the ARCP setting with the supervision of a Registered Nurse/Executive Director. Responsible for ensuring continuity of care of the residents by providing person centered service plans and ADL/Task sheets to the Care Partners, as well as supervising the care provided by nurses, Care Partners and supportive staff members.  Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facility’s mission.  

DUTIES AND RESPONSIBILITIES

Demonstrates Competency in the Following Areas:

   
  • Directs and supervises nurses and Care Partners. Develops schedules including vacation and holiday schedules, shift and work assignments. Ensures adequate staffing and expedites work; delegates administrative authority, responsibility and accountability to nurses/care partners as deemed necessary to perform assigned duties

   
  • Interviews and screens residents for admission.  Assists with resident assessments on admission, and when conditions change.  Develops and implements person-centered service plan, in compliance with federal/state guidelines, as well as ADL/Task sheets, and updates as required at 30 days, quarterly, and as needed, to provide optimal care for the residents.

   
  • Administers medications and follows the seven (7) medication rights and reduces the potential for medication errors.  Ensures medications are administered as prescribed by MD. Ensures physician’s orders are carried out in a timely manner.

   
  • Assists in planning, developing, implementing, and maintaining resident discharge procedures and plans; develops and implements a resident care organizational structure; participates in surveys and inspections by government agencies.  Reports to and makes recommendations to the Executive Director concerning operations of the department.      

   
  • Ensures that all resident care staff have valid and current licenses / certifications/ orientation and are performing duties in accordance with acceptable standards.  Ensures that all resident care staff attend and participate in mandatory inservices. Reviews staff complaints and grievances and ensures that all are treated consistent with policies and applicable laws.  

   
  • Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors 

   
  • Supports and maintains a culture of safety and quality.

   
  • Meets current documentation standards and policies.

   
  • Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.

   
  • Follows through as needed on information given by shift report, resident or family regarding resident concerns.

   
  • Implements plan of care of the resident based on assessments and goals established by the interdisciplinary care team.

   
  • Maintains residents' confidentiality and privacy; treats residents with kindness, dignity and respect; knows and complies with Residents’ Rights and ensures that all resident care partners know and comply with Residents’ Rights.  Monitors care services to ensure that Residents’ Rights and needs are met.  Reviews, promptly reports to the Executive Director, and maintains written records of all resident complaints and grievances.

   
  • Ensures that all resident care partners are aware of the plan of care.

   
  • Recommends to the Executive Director, the equipment and supply needs for the Resident Care Department; assists in developing and monitoring adequate and cost effective inventory control procedures.

   
  • Attends and participates in various committees and meetings of the community.

   
  • Provides written and/or oral reports pertaining to resident care, including incident reports, to Executive Director and others as required.

   
  • Follows procedures for reporting hazardous conditions or equipment; ensures that resident care associates follow established safety and infection control procedures when isolation precautions become necessary.

   
  • Ensures cleanliness of work areas.

   
  • Ensures that related resident care expenses are within budget

   
  • Participates as a Manager On Duty during assigned weekends.  Takes call for resident – oriented situations.

   
  • Any other tasks, assignments, projects or requests as deemed by management.

   
  • Treats residents and their families with respect and dignity.  Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families.

   
  • Supervises other members of the healthcare team as needed to accomplish the plan of care which has been developed.

   
  • Demonstrates competence in technical and manual skills according to scope of practice for RNs/LPNs/LVNs.

   
  • Demonstrates creativity in solving problems and looking at situations in new and innovative ways.

   
  • Interacts with residents, families, co-workers, managers and other departmental staff in a kind, knowledgeable and friendly manner.

   
  • Consistently involves resident and family in planning for care, treatment, and teaching needs.

   
  • Functions as a leader for the Care Partners; delegates in a manner that contributes to the excellence of the ARCP.

   
  • Participates in orientation of new staff and assists in performance evaluations when needed.

   
  • Demonstrates ability to remain calm in emergency and/or stressful situations and gives everyone a feeling of confidence that station is under control.

   
  • Consistently follows the policies and procedures of the facility.

   
  • Exhibits genuine interest in and appreciation of adults in later years of life.

   
  • Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.

   

 

Professional Requirements:

   
  • Adheres to dress code, appearance is neat and clean.

   
  • Completes annual education requirements.

   
  • Maintains regulatory requirements.

   
  • Maintains resident confidentiality at all times.

   
  • Reports to work on time and as scheduled, completes work within designated time.

   
  • Wears identification while on duty.

   
  • Attends annual review and mandatory inservices, as scheduled.

   
  • Attends/Assists with monthly staff meetings.

   
  • Represents the organization in a positive and professional manner.

   
  • Actively participates in performance improvement and continuous quality improvement (CQI) activities.

   
  • Complies with all organizational policies regarding ethical business practices.

   
  • Communicates the mission, ethics and goals of the facility.

   
    

Regulatory Requirements:

  • Graduate of an accredited RN/ LPN/LVN program.

  • Current Licensed Registered Nurse/ Licensed Practical Nurse/Licensed Vocational Nurse in State of  Louisiana

  • Current BCLS certification.

Language Skills:

  • Able to communicate effectively in English, both verbally and in writing.

  • Additional languages preferred.

Skills:

Basic computer knowledge.
