What are the responsibilities and job description for the Hospice Chaplain - Per Diem position at All Care?
POSITION SUMMARY: The Hospice Chaplain is a theologically and clinically trained professional responsible for the provision of spiritual care services to patients and families/caregivers of All Care Hospice, either directly or through coordination of care with other clergy or spiritual counselors. Hospice experience preferred. This position would service our entire geographical area.
The days/hours of this position are Thursday and Friday 8:30-4:30pm
ESSENTIAL DUTIES AND RESPONSIBILITIES
Assures spiritual assessment of patients, families/caregivers in the hospice program and assures appropriate services are rendered in a timely manner.
Provides direct spiritual care (interfaith) to meet the identified needs of patients and families/caregivers including bereavement support.
Maintains records of spiritual care services and communication with community clergy and related activities for quality assurance, program development as well as policies and procedures review and revision.
Provides consultation, education and support to the interdisciplinary group on spiritual care and bereavement. (One to one counseling, referral to community resources as needed)
Provides for Services of Remembrances for Skilled Nursing Facilities, Hospitals and Assisted Living Communities as requested and presides at/plans funeral and memorial services at family request.
Serves on Planning Committee (and participates in) annual All Care Hospice Service of Remembrance to meet the needs of personnel, volunteers and bereaved families and friends of All Care Hospice.
Attends hospice related events at the Agency.
Any other task assigned by supervisor.
QUALIFICATIONS
Experience, Education and Licensure
Master of Divinity (MDiv) or Masters in Pastoral Counseling or Theology from
an accredited Seminary/School of Divinity or 5 years experience as a Hospice Chaplain with proof of ordination as interfaith minister. Should have CPE, at least one unit. Experience in working with patients and families/caregivers providing spiritual care during serious illness, dying and grief. Hospice chaplaincy experience preferred. Must be familiar with MS Word, Outlook and Excel. Unless otherwise agreed upon by employer, employee must have a valid drivers license. Furthermore, employee shall have use of a vehicle during working times and insurance as a driver of that automobile.
