Job Description
We are currently looking for a phenomenal LPN for our home health agency. All HomeCaring, Inc. is a Medicare Certified, Twin-Cities-based, family-owned agency that is excited to be growing our territory in the Minneapolis.
The position consists of 20 Skilled Nursing weekly visits and some Office hours to be a nurse liaison.
Hours: Monday - Friday 8am - 4:30pm

Essential Job Functions/Responsibilities
- Provides direct patient care as defined in the State Nurse Practice Act
- The LPN furnishes services in accordance with agency policies, prepares clinical and progress notes, assists the physician and registered nurse in performing specialized procedures.
- Implements Plan of Care initiated by the registered nurse.
- Report any changes in the patient's condition to the registered nurse with the reports documented in the clinical record; provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participates in coordination of health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the registered nurse and/or Clinical Supervisor.
- Uses and/or prepares equipment, materials and supplies for treatments effectively and efficiently adhering to aseptic technique and assist the patient in learning appropriate self-care techniques.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.

- The LPN completes start of care's for Homemaking clients
- The LPN will complete Homemaking supervision visits every 6 months.
Position Qualifications
A person who is licensed as a Practical/Vocational Nurse by the State in which practicing .
Requirements
- Pass DHS Criminal Background Check
- Covid-19 Vaccine recommended but not mandatory
- Valid LPN License
- 2 Step TB Skin Test (We can provide)
- Valid Driver's License and Insurance
- Must complete company orientation and training
