What are the responsibilities and job description for the LVN position at All State Hospice Care, Inc?
Hospice is looking to hire a LVN who can go and see patients
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living
- Collaborate with healthcare team members to develop and implement patient care plans
- Perform nursing procedures such as wound care, IV insertion, and medication administration
- Document patient information, including assessments, interventions, and outcomes
- Educate patients and their families on healthcare management and self-care techniques
- Assist with pain management and symptom control for patients with chronic conditions or in palliative care
- Provide health coaching and support for patients in managing their conditions
Experience:
- Must be a licensed vocational nurse (LVN)
- Experience in providing specialized care for patients with dementia or other cognitive impairments
- Familiarity with hospice care principles and practices
Skills:
- Proficient in IV insertion and medication administration techniques
- Ability to provide inhalation therapy as prescribed by healthcare providers
- Strong communication skills to effectively interact with patients, families, and healthcare team members
- Excellent organizational skills to prioritize tasks and manage time effectively
- Compassionate and empathetic approach to patient care
Job Types: Full-time, Contract
Pay: $21.00 - $60.00 per hour
