We are currently looking for a Full time Social Worker to join our team.

Social Worker - Responsibilities:
- The Social Worker will assist with plans for psychosocial needs of residents in the Centers for Living and Rehabilitation.
- This position will likely participate in care plan meetings and document related resident emotional and social needs.
- Promotes and protects the rights of each resident and patient from admission through discharge.
- Collaborates with teams for coordination of community services.
- Assists residents and families in financial, legal, and social services within the community.
Social Worker - Requirements:
- Bachelor’s degree in social work required.
- Two years of social work experience in a health care setting, working directly with individuals preferred.
