Job Summary
The Chronic Care Manager is a Certified Medical Assistant who supports the development of patient-centered, team-based care. Supports our award winning team of physicians in managing their panel of patients using a population management informatics & communication tool. Works to identify patients’ unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and their care team, by proactively engaging with patients over time. Facilitates high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.
Essential Duties and Responsibilities-ASC
  • Develops a keen understanding of chronic pain and primary care practice requirements for optimal, coordinated population health
  • Works as an effective team member of the care team
  • Facilitates a Chronic Care Management platform to support patients with multiple chronic diseases and assists in the coordination of the patients care.
  • Manages patient registries and provides the members of health care teams, in designated practices, the data required to meet the health needs of the patient
  • Contributes to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, coaching and support, as well as referral to appropriate clinical staff when necessary
  • Contributes to the teamwork within and between departments. Regularly attends and participates in meetings with coworkers and practice staff.
  • Performs all job functions in compliance with applicable federal, state, local and company policies and procedures

Education and/or Experience
  • Must maintain certification or license at all times during employment
  • High school diploma or equivalent required
  • Graduate of an accredited Program
  • Must have at least 1 year experience in a medical office
  • Pain management experience preferred
  • Strong organizational skills with great attention to detail
  • Working knowledge of medical terminology, and correct spelling of medications
  • Strong grammatical skills
  • Proficient on computer and typing
Communication Skills
Strong customer service skills. Ability to maintain good relationships with patients, providers and coworkers. Ability to effectively give and receive information from patients, providers and other members of Alliance’s care team.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to manage time efficiently.
Computer Skills
Must have the ability to become proficient with the eClinicalWorks EMR system. Prior experience with an EMR required. E-Clinical works, preferred.
