What are the responsibilities and job description for the Case Manager position at Allied?
Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Coordinate and monitor client progress towards goals
- Provide crisis management and intervention as needed
- Collaborate with other professionals, such as behavioral health specialists and addiction counselors, to address client needs
- Assist clients in accessing resources such as Section 8 housing
Qualifications:
- Experience in case management or related field
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent organizational and time management skills
