Job Description:
- Conduct and coordinate the development and completion of the resident assessment (MDS)
- Coordinate resident’s discharge planning in conjunction with the therapy staff and Social Services.
- Coordinate scheduling the resident assessment and care plan meetings with the residents, family, and other pertinent staff members.
- Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
- Participate in the development and implementation of resident assessments (MDS) and care plans, including quarterly and annual reviews.
- Update 802 & 672
- Ensure managed care insurance companies are updated timely and all information is provided
- Initiating all certifications for Medicare and managed care patients
- Issuing NOMNC (Notice of Medicare Non-Coverage) & filing insurance appeals
- Attend and prepare for weekly Medicare meetings
- Prepare PT/OT/ST Evaluations
- Restorative Nursing Assessments, programs of care, quarterly notes
- New Employee Orientation
- TB tracking
- All other duties as assigned by Director of Nursing & Administrator
- Must have excellent leadership and communication skills
- Must have IL RN or LPN license
- Must have long term care experience
- Clear understanding of MDS requirements
- Must know MDS 3.0
- Knowledge of ICD-10 Coding
- Knowledge of Restorative Nursing
