Care Manager Job Description: The care manager is responsible for assessing and assisting patients, family members and care givers with their medical conditions and social and economic needs. The goal of care management is to reduce medical costs by improving patients’ overall health status through self-management, education, problem solving, and community resource referrals.
The care manager will work as a member of a multi-disciplinary care team.
Care Manager Job Duties:
Identify patients in need of assistance, be it self-management through education, recent significant life event, provider referral, staff referral, patient request.
Contact and enroll patients in Chronic Care Management or Episodic Care Management. Manage patient as primary point of contact throughout their participation in either program.
Create, in co-operation with the provider, patient, family member and/or care giver, a patient centered care plan with realistic and achievable goals. The care manager and patient will monitor and update the care plan as needed and establish new goals or care plans as needed.
Monitor and contact all patients discharged from hospital, short term rehab, skilled nursing facilities and emergency departments. Assist with any outstanding needs, ensure that patients schedule and attend any appropriate follow up appointments within the practice and with specialists.
Attend care team meetings with other members of the multi-disciplinary team. This includes meeting with Alma Family Practice providers and staff, family meetings and meetings with other agencies outside of Alma Family Practice, PC that provide care for the patient.
Prepare and direct group educations sessions that are held at AFP.
Monitor different reports and registries to help identify potential patients that could benefit from care management.
Assist physicians with comprehensive medication management appointments.
Assist in obtaining medical equipment as appropriate.
Attend, participate in and/or conduct staff-development programs.
Obtain continuing education as required.
Attend all training seminars that are required for incentive program participation.
Maintain clinical licenses and certifications as required.
Incentive Administrative Team member. Attend all meetings, webinars, and conferences as required.
Any other additional duties and responsibilities as required by practice manager, providers, or incentive programs within the care manager’s scope of practice.
Small amount of travel within the state of Michigan is required.
Other duties as assigned.
Skills and Knowledge:
Previous care management in a primary care or hospital setting is preferred.
Must be able to function within a team for problem solving and collaboration.
Previous EMR experience preferred.
Computer skills are necessary to accurately document all work in electronic medical records.
Interpersonal communication skills necessary to communicate with patients and caregivers via phone and in person. Professionalism is of great importance to provide quality patient care.
Well organized individual that can multi task under strenuous conditions.
Ability to prioritize daily job duties that will provide quality patient care in an opportune manner.
Must be capable of functioning independently and within a team.
Educational Requirements:
Graduation from an accredited college or university.
Associates degree in nursing.
Current and unrestricted license in the State of Michigan.
Basic Life Support certification.
