LPN Visit Nurse

Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family - Marinette PD. As a LPN, you will provide high-quality clinical services to patients in their homes, working under the supervision of a Registered Nurse.

Key Responsibilities

• Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other healthcare team members.
• Document patient visits thoroughly and accurately in the electronic medical record.
• Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
• Ensure confidentiality of patient health information.

Requirements

• Current LPN licensure in the state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification.
