Responsibilities:
- Collaborate with other healthcare professionals to coordinate care and ensure holistic treatment
- Assist clients in accessing community resources and support services
- Maintain accurate and up-to-date client records and documentation
Qualifications:
- Bachelor's or Master's degree in Social Work or related field
- Valid state licensure or certification as a Social Worker
- Strong communication and interpersonal skills
- Ability to work effectively as part of an Interdisciplinary team
- Compassionate, empathetic, and non-judgmental approach to client care
