The Social Worker will work collaboratively with clients to assess their needs, develop care plans, and connect them with appropriate services and resources.
Key Responsibilities
- Conduct comprehensive assessments of clients' needs, strengths, and goals.
- Develop and implement individualized care plans in collaboration with clients and other professionals.
- Provide counselling and emotional support to clients and their families.
- Advocate for clients' rights and access to necessary services and resources.
- Collaborate with community organizations, healthcare providers, and other stakeholders to coordinate services.
- Maintain accurate and confidential client records and documentation.
- Monitor and evaluate clients' progress and adjust care plans as needed.
- Participate in multidisciplinary team meetings and case conferences.
- Stay informed about current social services policies, programs, and best practices.
- Valid state licensure or certification as a Social Worker (LCSW, LMSW, or equivalent).
- Minimum of 3 years of experience in social work, preferably in a similar setting.
- Strong interpersonal and communication skills.
- Ability to build trust and rapport with clients from diverse backgrounds.
- Excellent problem-solving and critical thinking abilities.
- Knowledge of social services systems, community resources, and relevant legislation.
- Ability to work independently and as part of a multidisciplinary team.
- High level of organizational skills and attention to detail.
- Proficiency in electronic health records and Microsoft Office Suite.
- Bachelor’s or master’s degree in social work (BSW or MSW) from an accredited institution or a related field.
- Experience working with specific populations such as children, the elderly, or individuals with mental health or substance abuse issues.
- Bilingual or multilingual abilities.
- Additional certifications in specialized areas (e.g., clinical social work, trauma-informed care).
