Job Posting for Director of Clinical Services Home Health at Alternative Home Health Care, LLC

To provide quality clinical care in the home setting and to provide direct supervision to staff. Ensures that patient care delivery is coordinated between services in order to meet patients’ needs and to involve the patient, representative and caregiver(s) as appropriate, in the coordination of care activities. Participates in surveys made by authorized government agencies. Participates in Agency QA/PI activities as a member of the Quality Assessment/Performance Improvement Committee . Shares schedule with the Administrator to provide administrative and clinical support during all operating hours of the Agency.Available to see patients for staffing shortage or in emergency situations.Will participate in On Call rotation as assigned
