What are the responsibilities and job description for the LPN Home Health visits position at Altimate Care LLC?
We are seeking to hire qualified Licensed Practical Nurses (LPN) to peform Home Health skilled visits in the greater Cleveland area. Either on the West side or East side, you choose your territory! Under the direction of a Registered nurse, the LPN will manage patient care according to the patient's care plan.
Job Duties:
- Monitor vitals, perform skills and assessments
- Manage patient care according to the care plan
- Work with family members to instruct on disease management as needed
- Communicate changes in condition timely and accurately
Requirements:
- Prior Home Health care experience is a plus (will train)
- 1 year prior LPN home health care experience (Shift or per visit)
- Valid nursing license
