Overview:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality care to patients in the comfort of their own homes.
Responsibilities:
- Conduct patient assessments and provide skilled nursing care
- Monitor vital signs and symptoms, and report any changes to the healthcare team
- Develop and implement patient care plans in collaboration with physicians and other healthcare professionals
- Ensure compliance with HIPAA regulations and maintain accurate medical records
- Coordinate with interdisciplinary teams for comprehensive patient care, including case management and discharge planning
Requirements:
- Valid RN license
- Proficiency in patient assessment, care planning, and case management
- Strong communication skills and the ability to work independently
- Knowledge of home health regulations and best practices

Pay: $106,351.00 - $139,000.00 per year
Expected hours: 40 per week
