Job Summary
PRN homecare field work. Enable senior citizens to live comfortably at home and bring peace of mind to family members knowing their loved ones are cared for. Visit clients in their homes to conduct initial home care assessments and re-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding the benefits of home care. Conduct supervisory visits of Caregivers on-shift at client homes. Update care plan and supervisory notes in electronic records. New clients may at times require short notice assessments within the company service area of Fairfax County, Arlington, and Alexandria. Re-assessments and supervisory visits may be scheduled in advance. Reports to the Administrator. Assist and advise the Administrator with managing in-home non-medical care of existing and new clients to include in-home re-assessments, care plan update recommendations, and Caregiver supervision.
Job Responsibilities
- Conduct initial in-home assessments for new clients, coordinate care options with new clients and their family members.
- Conduct periodic in-home re-assessments of existing clients to ensure care plan meets any changing needs of the client. Coordinate changes in the level of care with Administrator.
- Conduct supervisory visits of Caregivers on-shift with clients at their homes.
- Occasional coordination with facility staff for clients who reside in facilities.
- Advise Administrator on levels of care and Caregiver training recommendations.
- Future opportunities for part-time or full-time as company grows
Required Skills & Qualifications
- RN with current license to practice in Virginia
- Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable.
- Highly proficient communicator, comfortable with various communication methods including phone, email, Zoom, etc.
- Reliable transportation and willing to travel to client homes for assessments and Caregiver oversight.
- Experience with electronic client record management systems.
- Flexibility and willingness to meet the unique needs of clients, caregivers, and family members.
- Authorized to work in the United States for any employer
- Consent to a Background Check and Drug Screen
Salary : $35 - $55
