This Regional Care Consultant is responsible for providing care consultation and education services for the counties in the designated service area of operation including Dickinson and surrounding communities around southwestern North Dakota. Responsibilities include travel to designated regions for purposes of implementing community and care partner education, provision of care consultation on dementia-related clinical / family issues, and development and maintenance of medical primary care provider relations and the assistance in the development of informational resources. This position will fulfill requirements of a contract with the North Dakota Department of Human Services including accurate and timely documentation, development and facilitation of care consultation services and trainings and tracking referrals.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
* Provides Care Consultation services to individuals with dementia (any stage) and their care partners. Care consultation may consist of assessment of needs, identification of issues/concerns and available resources, development of a plan of care, referral to appropriate resources, provision of emotional support and education about dementia, and follow-up, as needed. Care consultation services may be provided by phone, or in person through individual and family meetings
* Delivers community/family and professional education programs
* Performs outreach activities in the community and in health care systems to increase referrals to the Association
* Discusses challenging cases with supervisor, as appropriate, and works collaboratively with other Care Consultant staff
* Maintains appropriate documentation in database
* Increases the number of people with early dementia who access Alzheimer’s Association products and services
* Participates in regular clinical staffing, group/individual supervision, and continuing education sessions
* Recruits and co-manages volunteers who deliver education and outreach activities
* Travels to Dickinson and surrounding communities in southwestern North Dakota as well as to the Bismarck-Mandan area to deliver education, care consultation, and outreach
* Represents the Association in media interviews, as needed
* Participation in other Alzheimer's Association department events to provide education and awareness

QUALIFICATIONS:
* Minimum four year degree in social work, nursing (or a diploma RN or LPN), counseling, education, occupational therapy, psychology, sociology, therapeutic recreation, or related human service field. A Master’s degree in counseling, social work, nursing, or psychology is preferred, or Master’s degree equivalent experience, especially in the areas of dementia care or elder services may be considered
* Three years experience working in the geriatric field and the provision of psycho-social assessments and related action planning
* Experience working with diverse communities, preferred
* Knowledge of Alzheimer’s disease and related disorders
* Experienced clinical/counseling skills in listening, assessment, empathy, advocacy, negotiation, referral, and closure
* Professional experience conducting health evaluations, diagnostic intakes, and/or crisis intervention counseling
* Strong presentation skills and previous experience as a public speaker or educator
* Strong organizational, staff and project management skills
* Knowledge of community resources
* Willingness to work on evenings and weekends, if necessary
* Ability to operate a motor vehicle. Must be in possession of a valid driver’s license and insurance
* Ability to travel on frequent basis
