What You'll Do:
- Receive referrals by phone, fax, email, or in person for designated department
- Provide explanation of the benefits to patients, families, and community contacts
- Oversee scheduling of initial Skilled assessment and admission
- Maintain admission packets and ensure that proper forms are available to admissions staff
- Knowledgeable in all Home Health processes, with basic knowledge of Hospice and Personal Care Services processes.
Qualifications:
- Be eighteen (18) years of age or older
- At least one (1) year experience in a homecare, hospice and/or healthcare related field preferred
- Valid driver's license and proof of insurance is required
