JOB SUMMARY:
The Hospice aide, under the direction of a Registered Nurse, provides personal care within the guidelines of the interdisciplinary team Care Plan to patients
QUALIFICATIONS:
- Be eighteen (18) years of age or older
- Certified in the State of New Mexico to provide Hospice aide services
- Successful completion of Ambercare Hospice aide competency exam and skills demonstration
- Must be able to perform physical demands of patient care including lifting and transferring of patients
- Must have a valid NM Driver's license
- Must have reliable transportation available during work hours. Vehicle must be properly registered and insured in accordance with state law.
- Must possess current CPR, First Aid Certification, CNA Certification in the State of New Mexico
DUTIES AND RESPONSIBILITIES:
- Provide essential personal care assistance to patients consistent with policies and procedures of Ambercare.
- Document care given in accordance with policies and procedures of Ambercare
- Report any changes in patient condition to RN or Director of Nursing
- Attend at least 12 hours of in-service training per year
- Attend and participate in interdisciplinary team meeting as requested
- Provide emotional support and compassionate care to patients and families
- Demonstrate listening skills and offer encouragement when appropriate
- Practice universal precautions when caring for patients
- Provide incidental household services as are essential to the patient's well-being at home.
- Take and record vital signs as directed by the Nurse Case Manager
- Adhere to dress code
- Adhere to work schedule and maintain good punctuality and attendance record E-210
REGULATORY REQUIREMENTS:
- Satisfactory completion of pre-employment health screening if applicable, including drug screening.
- Satisfactory completion of background check in accordance with State of New Mexico Caregivers Criminal History Screening Program.
PHYSICAL OR ENVIRONMENTAL DEMANDS:
Heavy - Must be able to lift 75 lbs or greater alone or with assistance for patient transfers. Frequent pulling, pushing, bending, stooping, reaching
