Amberwoods is a 130-bed facility providing long-term skilled nursing, short-term rehabilitation and Memory Care services, located in Farmington, CT. Our facility is conveniently located within proximity to I-84 and is a ten-minute drive to downtown Hartford..
- Demonstrates patience, a positive attitude, ambition, and being a team player.
- Complete required EMR documentation in a timely manner including assessments.
- Hold Resident Care Plan Conferences with the interdisciplinary team to review resident's plan of care.
- Assist the interdisciplinary team with any concerns that arise with patients and/or families.
- Support residents' psychosocial needs.
- Support residents, responsible parties and families during adjustment to placement.
Experience:
- Nursing Home experience in long-term-care preferred.
- Must have knowledge of the PASRR process and the federal/state rules and regulations.
Education Requirements:
Bachelors or Master’s degree in Social Work.
