Description
The Refugee Case Manager for the Health Screening/Promotion program(s) shall provide services as defined and stipulated by ACC’s contract with the LEO-OGM, using in-depth knowledge of various cultures, family values, social structure and language(s).
Refugee Health Assessment Program:
- ACC’s Refugee Health Assessment Program provides the initial health assessment for newly arrived refugees in Macomb, Oakland and Wayne Counties.
Refugee Health Promotion Program:
- ACC’s Health Promotion Program establishes a referral process to include coordination for provision of follow-up services needed as a result of the client’s health screening.
We are hiring 1 Case Manager:
- Bilingual fluency in Ukrainian and English
Qualifications:
- Bachelor’s Degree in the field of Human Services, Health related field or work equivalent
- Experience in working with people from different socio economic and cultural backgrounds
Case Manager duties & responsibilities:
Case Manager duties & responsibilities:
- Coordinate appointments between the doctors’ offices and the clients or referring agencies
- Assess client needs through a screening and intake process
- Provide or coordinate interpretation and translation services in English/and Native language.
- Assist clients in understanding Medicaid and US Healthcare system
- Maintain relationships and interface daily with appropriate referring agencies, program partners, clients and sponsors
- Act as a liaison between clients and referring agencies
- Appointment follow up with clients and referring agencies regarding any appointments or missing information or medical needs
- Verify Medicaid information for accuracy
- Maintain, with confidentiality, clients’ records and information
- Provide supervisor/administrator office with a monthly report accordingly
- Refer clients to the other services programs that are provided by ACC and partners
- Source and create culturally appropriate health education materials for common health needs
- Perform effectively within a team environment
- Attend all staff meeting as scheduled
- Any other duties as assigned by the Supervisor
