Clay Health Care Center is seeking an Unit Charge Nurse LPN/RN our Ivydale, WV location.

Clay Health Care Center joined the AMFM Family July 1, 2014 and is a 60 person care center specializing in skilled nursing, short-term rehabilitation to home, and long-term care services.
Summary: We are currently seeking a dedicated unit charge nurse who can direct the operations of patient services to ensure that the highest degree of quality patient care is always maintained in accordance with state and federal regulations, facility policies and procedures and patient directives. An ideal candidate must provide leadership for the staff in the department in accordance with approved policies and procedures.
Responsibilities:
  • Make patient and environmental rounds on a routine basis through shifts to ensure that patient care is in accordance with acceptable nursing standards and the plan of care for the patient.
  • Assure compliance rounds are complete in a timely manner per shift.
  • Identify patient problems and complaints and report them immediately to the appropriate discipline.
  • Assure documentation is complete, accurate, legible, informative, and consistent with standards of care.
  • Accurately input documentation into kiosk. Retrieve and review designated reports as needed.
  • Provide supervision and direction of dining experience.
  • Assure patient participation in facility activities and manage staff accordingly.
  • Assist new employees in following facility processes, policies, and procedures.
  • Always respect the dignity and privacy of individual patients.
  • Thorough knowledge of restorative devices and equipment to provide patient care.
  • Perform treatment and procedures within scope of practice.
  • Administer and record medications as prescribed by physician and in accordance with applicable laws and regulations.
  • Accountable for the direct observation and counting of each narcotic drug at the start and end of duty.
  • Responsible for maintaining and recording prescription medication and over the counter medication.
  • Present a positive image and accept the need for changes, be flexible and demonstrate enthusiasm toward enhancement.
  • Consistently work cooperatively with patients, patients’ representatives, facility staff, physicians, consultants, and ancillary service providers.
  • Listen to patient and family complaints and report problems; take appropriate action or report to supervisor.
  • Motivate and communicate with the nursing workforce assuring minimal turnover.
  • Effectively manage the nursing employee satisfaction rates to assure high employee morale.
  • Scheduled and actual worked hours for the nursing department are not to exceed budgeted hours allowed each month.
  • Assure that enough licensed nurses and Certified Nursing Assistants are available for each shift. Develop consistent work assignments across all shifts.
  • Provide competencies to Certified Nursing Assistants.
  • Oversee performance evaluations for C.N.A, HTA, NAT and assure timely completion of all performance evaluations.
  • Provide education and effectively implement progressive discipline process.
Education/Requirements:
  • Must be a graduate from an accredited school for registered nurses and must be licensed or eligible for licensure by the State of West Virginia.
  • License must be current, unencumbered, and active in the state of West Virginia.
  • Must possess and maintain current CPR certification, or acquire certification within 90 days of hire.
