About the Americares Free Clinics:
The mission of the Americares Free Clinics is to provide quality health care to low-income, uninsured patients in a setting where all individuals are treated with dignity and respect. Americares opened its first clinic in 1994 to expand access to care for the working poor. Over the years our program has grown to become the largest free clinic network in the state with four locations serving 3,000 patients annually. Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.
Summary:
The purpose of the Wellness Center is to guide people into a healthier lifestyle by providing health and wellness programming, as well as providing clients with advice and knowledge about services available in the community. S/he will help clients through support and reinforcement leading to a healthier lifestyle. The Wellness Center Coordinator is responsible for coordinating programs in accordance with operational, quality, safety, and service standards of Americares Free Clinics. S/he will oversee these programs and act as a liaison with our partner agency, Building One Community*, who will utilize the center for various programing such as pro-bono legal counseling, new immigrant support services, ELS classes and job training.

Duties and responsibilities
- Conduct initial client interview; identify and evaluate client needs; develop a client program to fit those needs as determined by the availability of programs at the Wellness Center and in the community

Coordinate the planning and delivery of creative and value-added wellness programs including health awareness (bulletin boards, newsletters), coaching and lifestyle programs, educational campaigns (lunch n learns, diabetes workshops), behavior change programs (Choose to Lose, Maintain Don’t Gain), health assessments/screenings (biometric events, incentive programs, health fairs), smoking cessation, and more.

Communicate and coordinate with partner organizations to promote, sustain, and expand wellness resources for clients (i.e. YMCA, fitness centers, nutrition/cooking resources)

Participate in community meetings to understand community issues, build relationships with community members, and to advocate for our clients
- Effectively promote and market the wellness programs to target populations, health departments, and hospitals.
- Research relevant topics in health promotion and disease prevention, as needed.

Prepare and maintain records, documentation and database support data for wellness program reporting to monitor effectiveness of the Wellness Program.

Ensure the Wellness Center policies are adhered to and waivers are completed by all clients participating in wellness program.
Ensures safety of participants and facility property.

Other Duties and Responsibilities
- Occasional travel may be required for investigating community resources and collaborations, training sessions, and other company functions.
- Responsible to complete all mandatory annual trainings as deemed necessary by Americares.
- Adherence to the Dress Code policy.
- Maintain current certifications and acquire new certifications as opportunities arise through professional improvement workshops and conferences focused on current affairs in wellness, exercise, health sciences.
- Maintain HIPAA standards and regard all medical or healthcare information pertaining to clients as confidential.
- Perform other duties as assigned by Americares Wellness Center initiatives.
Qualifications
- 3 years of experience working in a wellness/health promotion environment as a community health worker, patient navigator or health educator. Nutrition and/or fitness experience a plus.Certification a plus.
- Demonstrated cultural competency with Hispanic communities and others of varied ethnic, cultural backgrounds, ages and economic circumstances.
- MUST be totally bilingual in English – Spanish both written and oral.

Must possess the ability to move from concept to implementation, have demonstrated program coordination experience and have strong data collection and analysis skills.

Must possess knowledge of and ability to use computers with Windows-based business software applications. For example: Microsoft Word, PowerPoint and Outlook; Skills in oral and written communication, including the ability to draft, proof and construct professional documents and correspondence

Positive attitude and caring demeanor a must.
- Up to 20% local (Stamford) travel required; must work onsite not remotely
Working conditions
- Frequent contact with clients and outside agencies.
- Frequent exposure to communicable diseases and other conditions common to a Wellness Center environment.
- Requires adherence to Infection Control Standard Precautions.
- All the above is met within the standards of the Americares Free Clinics pandemic guidelines for safety.
Physical requirements
- Approximately 50% walking/standing and 50% sitting.
