Certified Nursing Assistant / Floater
Nurse Team Leader
POSITION SUMMARY:
Provide personal care to patients in the home setting, patient care unit, and/or nursing facility
EDUCATIONAL QUALIFICATIONS:
- High school diploma required
- Graduate of a Certified Assistant Training Program I
- Current listing on NC State Registry
- CPR certification
KNOWLEDGE AND SKILLS:
- Knowledge of and commitment to the hospice philosophy of care
- Ability to work on an interdisciplinary team
- Must be able to work up to (2) weekend shifts per month
- Must be able to work AM and PM shifts
- Willingness to work with patients/ families regardless of race, color, creed, age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness
- Exhibit positive attitude and morale with all team members
Initial And Ongoing:
- Complete Hospice volunteer/staff training program
- Complete Hospice orientation program
- Obtain 12 hours of continuing education per year, including mandatory safety in-service
- Attend minimum of six staff meetings per year
- Maintain CPR certification
DUTIES AND RESPONSIBILITIES:
(Home care)
- Assist in orienting new team members, as requested
- Participate in quality improvement program
- Adhere to strict confidentiality
- Adhere to safety policy and practices
- Complete employee time record accurately per policy
- Cooperate with all government agencies and outside auditors
- Assist with updating policies and procedures, as appropriate
- Complete annual TB screening
- Assure all license, certifications, continuing education information and any other required documentation are submitted to human resources and kept current
- Maintain up-to-date documentation required and specified in Hospice policy and procedure
- Perform other duties assigned by supervisor
- Follow all infection control procedures
- Assist patients in achieving maximum self-reliance in activities of daily living
- Assist with baths, mouth care, skin care, hair care, and back rubs
- Monitor vital signs, as directed
- Assist with ambulation
- Assist with diets/special nourishments
- Change bed linen, as needed
- Assist with prescribed exercises as instructed by appropriate professional personnel
- Actively listen to patient and family concerns and allow expression of their feelings
- Assist in patient care unit, home care and/or nursing facility as directed by supervisor
- Provide homemaker services:
patient’s clothing (laundering and ironing)
basic housekeeping tasks (sweeping, mopping, vacuuming, dusting, and dishes)
making or changing bed
preparing simple meal
assist with reading and writing
DUTIES AND RESPONSIBILITIES:
(Patient Care Unit)
- Answer call light promptly
- Take necessary safety precautions:
side rails up, as necessary
call light within reach
bed in lowest position
- Maintain patient’s room in a clean, neat, orderly manner with clear walking area
- Cook breakfast for Kirkwood patients, as requested
- Assure patient/facility laundry is appropriately cleaned and restocked
- Make for provisions for patient and family privacy at all times
- Speech and hearing skills necessary for telephone interaction and face to face contact
- Visual acuity to read chart and physician’s orders
- Ability to stand and walk for long periods of time

- With assistance, ability to lift and support an average of 150 pounds
- Ability to sit as required to perform duties
- Ability to pull/push, and bend frequently
- Ability to squat, kneel, climb, and balance
