Anchor Health Home Care is seeking a dynamic and compassionate individual to join our team as a Case Manager - Patient Support Specialist. As the face of Anchor for our patients in our HCBS Department, you will play a pivotal role in providing exceptional support to our patients and their families.
Responsibilities:
Communicate effectively with patients and their families to address any concerns or difficulties they may encounter on a daily basis.
Provide superior customer service and support in a compassionate manner.
Utilize strong organizational skills to prioritize and handle multiple tasks efficiently.
Manage projects by establishing objectives, milestones, and creating necessary documentation.
50% travel required
Work autonomously while also collaborating effectively with the team.
Maintain professionalism and a positive attitude in high-pressure environments.
Qualifications and Experience:
Bachelor's degree is required for this role.
Minimum 1 year of customer service experience.
Proficiency with computers, including Microsoft Word and Excel.
Ability to learn and operate scheduling and training software systems.
Highly detail-oriented with excellent time management skills.
Healthcare experience preferred; homecare experience is a plus.
Bilingual in English and Spanish preferred.
