Anchor Post Acute is a beautiful 120-bed skilled nursing and Rehab facility conveniently located behind Aiken Mall in Aiken, South Carolina. The Social Services Director should be a skilled communicator, director, leader and motivator and who has a passion to provide for each resident’s social, emotional and psychological needs. Successful candidates will have:

• Bachelors Degree in social work OR
• Bachelor’s Degree in a human-services related major and with 1 year in healthcare setting where the individual is working as a supervised social worker who works directly with individuals.
• Must be able to organize and prioritize many tasks effectively
• Ideal candidate must be a skilled communicator, director, leader and motivator

Duties & Responsibilities:

• Responsible for planning, developing, organizing, implementing, evaluating and directing of the social service programs of the facility
• Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents
• Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required
• Handles pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, proactive, listening, verbal communication, and resident and their family relationships.

Pay: From $60,000.00 per year
