Job Title: Home Health RN, Farmington and Surrounding Territories
About the Role:
We are seeking a skilled and compassionate Home Health RN to join our team at Andwell Health Partners. As a Home Health RN, you will provide high-quality nursing care to patients in their own homes, working under the direction of a physician's plan of care.
Key Responsibilities:
- Develop and implement individualized plans of care that address the patient's holistic health care needs.
- Provide education and support to patients and their families to promote informed decision making and self-care.
- Assess and identify the patient's health care needs, and develop strategies to address them.
- Collaborate with the patient's healthcare team to ensure seamless care coordination.
- Document patient progress and outcomes in accordance with regulatory requirements.
Requirements:
- Current R.N. license in the State of Maine.
- ANA certification in Home Health or Community Health Nursing encouraged.
- Current CPR card required.
- One year of recent clinical nursing experience required.
- A reliable car with insurance.
