What are the responsibilities and job description for the Patient Navigator position at AngleZ Behavioral Health Services?

AngleZ Behavioral Health Services is currently seeking a Patient Navigator to join our Opioid Health Home team.

The Opioid Health Home operates with a team-based approach to care, which includes prescribers, counselors, and recovery coaches. The patient navigator will work with members of the OHH program to collaborate with other health care, mental health, social service, and community providers to guide members in accessing additional services and support that will help aid in their recovery.

The Patient Navigator shall be the primary provider of care coordination, health promotion, individual and family support services, and referral to community and social support services. In the instance that the member also receives additional coordinated case management services, the patient navigator will provide support to the care coordinator and member as needed. The patient navigator will have an active role in the treatment planning process, will provide clear and accurate documentation to community resources as well as in an electronic health record, and will need to have strong communication and problem solving skills.

Must have one of the below:

  • Have at least one year of job experience in a health/ social services or behavioral health setting and hold an Associates degree.
  • Be a MHRT/C with at least one year of related work experience
  • Have a bachelors degree from an accredited four year institution of higher learning
  • Be a Community Health Worker (CHIW)

Job Type: Full-time

Pay: $20.00 - $23.48 per hour

Expected hours: 40 per week
