I. SUMMARY OF POSITION PURPOSE
Provides personal care and incidental household services to assigned patients in their place of residence under the direction and supervision of a Home Health RN or Therapist.
II. SPECIFIC DUTIES AND RESPONSIBILITIES
- Performs personal care and incidental household services for each assigned patient according to the written directions completed by the RN or Therapist.
- Reports appropriate concerns to RN or Manager
- Maintains current clinical record documentation regarding services provided to each patient.
- Utilizes effective communication skills to promote excellent customer satisfaction, patient care, and unit operations.
- Maintains expected productivity standards.
- Participates in home health agency functions as required by agency policies as well as accrediting and regulatory organizations.
- Identifies learning needs and assumes responsibility for seeking out opportunities to meet general and unit specific competencies.
- Other duties as assigned.
III. EDUCATION / QUALIFICATIONS
Minimum Educational Level Required :
- High school diploma or GED certificate preferred
- Must have ability to read, write and execute instructions.
Length and Type of Previous Job Experience Required :
Previous nursing assistance experience preferred.
Professional Certifications Required :
Nursing Assistant Certificate required.
Other Special Training / Competencies Required :
- Home health aide training and competency evaluation or competency evaluation is required.
- Must successfully complete the agency’s competency evaluation.
- Must be capable of assisting with activities of daily living and following a care plan with minimal supervision.
- The ability to provide reliable transportation as required by the nature of the job.
