AAHAA Network Nurse
Bring your nursing experience to this amazing part-time opportunity. You will work one on one with clients helping them achieve better health by focusing on improving their access to healthcare/social services and providing health education, advocacy, and referrals.
Your primary day to day function is speaking with individual clients, performing need-based assessments, and creating customized care plans which improve both their access to healthcare/social services, and their understanding of the process. Then you provide the follow-up ensuring their needs have been met. You will establish strong nurse/client relationships and develop solid working relationships with other community providers.
Essential Job Functions
- Develop trusting respectful relationships with clients
- Create individualized care plans improving clients’ health education, access to care, and general well being
- Work collaboratively with medical providers, social service organizations, and community organizations to promote services, identify new clients, and ensure existing clients are receiving the help they need
- Utilize excellent charting skills tracking your interactions/interventions with clients
- Report on the impact of your activities and client feedback
- Identify new partners that can provide services for our clients
- Conduct occasional community based educational programs
- Represent the organization at identified events consistent with the organization’s mission
Requirements
- Bachelor of Science in Nursing
- Current IL Nursing License
- Strong interpersonal communication skills
- Case Manager/Home Health/Community Health experience highly desirable
Other Skills/Abilities
- Proficient with Microsoft Word, PowerPoint and Google Applications
- Work well in an autonomous position
- Strong problem-solving skills
- Strong writing skills
- Comfortable with communicating to medium/large groups
- Bilingual a plus but not required
