SOR Coordinator – Conexio Care
Founded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexio’s original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. Our mission expanded in 1988 to include homeless services and substance abuse treatment.

The SOR Program Coordinator will be responsible for providing overall oversight of the SOR Grant, including Tier 1 and Tiers 2 and 3. The Coordinator will be responsible for supervising all grant staff and ensuring that all services are being provided per grant requirements. The coordinator will also be responsible for gathering and maintaining all data for grant reporting and ensure that all reporting of that data is completed in a timely manner per grant requirements.

Duties of the position:
- Supervision of all SOR grant staff.
- Provide education on the use of use of Motivational Enhancement Therapy (MET), Contingency Management (CM) and the Matrix Model for Intensive Outpatient Treatment for STUD and work with staff to expand their skills and increase the utilization of these evidence-based models with individuals with Co-Occurring disorders in the ACT and CRISP programs.
- Provide weekly clinical Supervision to work with staff to enhance their skills in working with individuals with SUDs.
- Ensure that staff are trained and are following the Contingency Management (CM) established for use with the SOR Grant.
- Ensure that all grant data is collected and compiled in a timely manner. This includes demographics, clinical metrics (Naloxone prescriptions and kits distributed), MET Clinical Supervision Training and SoS, Contingency management dated on which clients participated in the CAM and amounts and types of rewards received and their attendance at the targeted activities., Number of staff trained on the matrix model, staff develop metrics indicating proficiency in Motivational Enhancement Therapy, Contingency Management and Matrix Model.
- Ensure that all required data reporting is completed per grant requirements.
- Coordinate SOR Grant efforts with the Directors of ACT and CRISP Services, the ACT Clinical Supervisor and ACT and CRISP Team Leads
Required Education and Qualifications:
- Experience in working with grants.
- Bachelor’s degree in psychology, counseling, human services, social work, nursing, rehabilitation or related field from an accredited college or university OR Highschool Diploma
- 5 years of experience in the field. Mandatory 40 hours or more in 40 hours of education or CEU’s in substance abuse, preferably has a CADC certification.
- Experience working with persons with severe mental illness and/or substance abuse.
- Experience in data collection and evaluation required.
- Experience with underserved populations preferred
- Excellent communication skills and organizational skills
- Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders
- Excellent writing skills and record keeping skills
- Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.
Additional Qualifications:
- Computer knowledge of MS Office, and other HR applications is a plus, not required.
- Possess strong verbal and written communication skills.
- Must be proactive, organized, and possess great follow-up skills.
- Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
