We have immediate openings for highly motivated and clinically talented Field Clinicians to treat patients in the field (in the home of the patient). As a Field Clinician with Apogee Health Solutions you have the flexibility to set your schedule and you can adjust your visits depending on what fits with your circumstances and lifestyle. The successful candidate will conduct brief but thorough non-invasive patient examinations in the home of the patient and provide Apogee Health Solutions with documentation in a timely and efficient manner (within 24-48 hours of the visit).

Responsibilities:
- Conduct brief but thorough non-invasive patient examinations in the home of the patient
- Interpretation of diagnostic tests and recommendation of appropriate follow up care - this includes but is not limited to labs, mental health services, home health care and/or other referrals as appropriate
- Educate the primary care physician, case manager or referring physician on patient status as needed
- Provide
Apogee Health Solutions with appropriate documentation in a timely and efficient manner (within 24-48 hours of the visit)
- To be able to perform all necessary duties you MUST PROVIDE YOUR OWN stethoscope, blood pressure cuff, and pulse oximeter
- Maintain accurate records of patient encounters
- Utilize a designated EMR database for documentation purposes
Qualifications:
- Nurse Practitioner graduate of an accredited school
- Current, active, unrestricted license as a nurse practitioner in the state
- Knowledge of health and patient care regulations
- Ability to practice autonomously
- Valid and unrestricted driver's license
- Must have malpractice insurance
- Excellent verbal and written communication skills
- Ability to set priorities while managing multiple tasks
- Detail oriented with excellent organizational skills
- Current and active PECOS Enrolled provider preferred
- Current and active MEDICARE PTAN (Enrolled provider) preferred
Experience: No experience required / New grads welcome to apply!
