Outreach Navigator position For Haven for Hope in Downtown San Antonio. This person needs to have outreach experience, preferably in the nonprofit sector.
- Manages and facilitates enrollment to Jail Outreach, which includes capturing and documenting client’s history of chronic homelessness and/or other program eligibility requirements.
- Refers newly enrolled clients to internal Clinical Services and Housing programs, as well as registers client with SA Homelink.
- Facilitates program meetings and staffing with enrollees and staff to further support clients in accessing services, with the goal of eliminating barriers to stable housing.
- Document and keep record of updates, progress, and assigned tasks for each client discussed in staffing.
- Provide case management and peer support services clients enrolled in JO Program
- Provides resources navigation to clients, which includes ID recovery services, Supplemental Nutrition Assistance Program, benefits attainment services and other financial sustainability programs, mental health services, recovery support, and other services that will facilitate client success.
- Maintain a supportive, recovery focused environment.
- Reviews rules and expectations of the program with clients during enrollment and as needed throughout the duration of their enrollment.
- Communicates regularly.
- Review and assure accurate data quality on a daily basis in HMIS.
