The Family Service Coordinator / Developmental Specialist working in the Early Intervention setting will perform duties case management for the team of professionals and therapists who provide services that provides services to the child in the FIT program. Additionally, the Family Service Coordinator/Developmental Specialist II will provide services to the child in the FIT program by completing home visits in the child’s natural environment working to achieve the developmental age-appropriate milestones. The Family Service Coordinator/Developmental Specialist II will support and communicate the progress of services and offer strategies to achieve the desired outcomes. The Family Service Coordinator duties of case management determines the child’s continued eligibility to remain in the program.
What we are looking for:
-FSC duties will manage the team of professionals that provide the services to the family as recorded on the IFSP.
-DI II will provide the developmental services to the child as recorded on the IFSP.
Meeting the EI program documentation requirements.
-Providing the face-to-face meeting after the evaluation is complete recommending possible services and communication with the families.
-Work collaboratively with all staff to provide exceptional family focused customer service.
-Maintains an IFSP document according to the NM FIT program guidelines.
-Maintains the monthly face-to-face communication requirements to communicate the child’s progress and addresses any family concerns.
-Initiates attends and manages the TTA Consultation meetings with the other team members who are also providing services to the child.
-All other duties assigned.
Requirements:
-Bachelor’s Degree or higher in the appropriate field of study designated by the FIT program.
-Completion of NM FIT program FSC and DI coursework which enables the employee to provide services to the child and complete billing in the EI program.
-Dependable, detail-oriented, team player with strong interpersonal skills; ability to facilitate a team approach toward treatment; provide guidance to staff on a variety of topics; and ability to effectively organize and communicate information to target audiences.
-Bilingual skills (English/Spanish) are required.
-Ability to work in a changing environment gathering relevant information to integrate a change or modify a program.
-Must be a team player.
-A strong advocate of the Aprendamos mission to help children with development disabilities achieve their greatest potential and be motivated to contribute to the mission with pride and integrity.
This position is Full Time.
*Aprendamos Intervention Team has mandated the COVID-19 Vaccine and booster. We require staff to be fully vaccinated and boosted or get fully vaccinated and boosted within their first 45 days from date of hire.
