Overview
Responsible for recreation therapy and socialization for PACE participants as part of the PACE interdisciplinary team. Plans, organizes and conducts a comprehensive program of individual and group recreation to enable participants to achieve optimal levels of independent functioning and socialization. Works under general supervision.
Responsibilities
- Develops and implements the recreational components of the Interdisciplinary team plan of care to promote socialization and independent recreation and maintain the participant’s optimum level of functioning.
- Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of independence. Remains alert to pertinent input from other team members, participants and family members/caregivers
- Provides direct social, musical and recreational activity as needed as part of the plan of care.
- Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
- Regularly informs the interdisciplinary team of the medical, functional, and psychosocial condition of each participant. Remains alert to pertinent input from other team members, participants and caregivers.
- Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to perform duties.
- Participates in PACE and ARCHCARE SENIOR LIFE meetings including but not limited to, interdisciplinary team meetings, family meetings, staff meetings, in-service and training programs, meetings and Quality Improvement activities.
- Manages assigned caseload and maintains productivity per program expectation; provides recreation services to participants as per plan of care.
Education Requirements
- HS diploma or Bachelor’s Degree in recreational or activities therapy, music therapy or other therapeutic activities area preferred.
Experience Requirements
- Two years of experience in a social or recreational setting providing and coordinating services for frail elderly populations within the last 5 years, one of which was full time in a patient activities program in a health care setting. Bilingual skills may be required, as determined by operational needs.
- Have 1 year of experience with a frail or elderly population
Certification
- Certified Therapeutic Recreation Specialist Preferred'
