Care Manager RN - Remote

This is a remote position with local travel; candidate must live in Northwest - Michigan.

The Area Agency on Aging of Northwest Michigan (AAANM) is the place to turn for assistance with complex issues facing older adults, adults with disabilities and caregivers. AAANM links to, coordinates with, and provides a variety of programs, services, and resources within the 10-county service area of northwest Michigan. AAANM is a nonprofit corporation operating as a designated Area Agency on Aging by the Michigan Department of Health and Human Services / Aging and Adult Services Agency and is a Medicaid MI Choice Waiver agent.

Job Summary: Care Managers use person-centered principles to assess the complex medical, functional, psychological, financial, and environmental needs of case management participants. The supports coordinator will work with the participant and their supports to develop a person-centered plan and arrange for needed services and supports. Advocate for participants and their allies and ensure the health, safety, and welfare of program participants. Aspects of this position will be conducted as part of a nurse/social work team.

Pay: Based on experience

Schedule: Monday through Friday 8am-4:30pm with some flexibility in work schedule. Position is remote based, however it does require some travel throughout the northwest Michigan 10-county service area and may include occasional trips to other parts of the State; Ability to work outside scheduled hours when need arise.

Essential Job Functions:

• Conduct in-person comprehensive assessments and reassessments of participants health status.
• Using a person-centered approach, develop a plan of care in conjunction with participant and all allies participant wishes to be involved in the planning process and participant’s physician.
• Seek clinical consultation from supervisory staff as needed to assure high quality plans of care.
• Monitor participant condition and provision of service pertaining to plans of care.
• Adjust plan as determined through reassessment or normal monitoring. Maintain communication with participant’s family and physician.
• Maintain HIPAA compliance and confidentiality at all times regarding participant and co-worker information per all agency guidelines.
• Maintain all Care Management procedures, including documentation standards, as established by AAANM.
• Be an advocate to continually seeks ways to improve process and procedures.
• Ensure the responsibilities of the position are always handled in a safe and responsible manner.

Education and/or Experience

• Registered Nurse (RN)
• Bachelor of Science in Nursing (BSN) preferred
• Prior care management experience preferred
