Home Health Nurse

Well established for more than 15 years Visiting Nurse Agency is looking for Registered Nurse to conduct home visits. We are looking for professional clinician who understands humanity and willing to help. Experience is not necessary, teaching provided.

Responsibilities and Duties

-Provide one-on-one care for patients in their homes

-Assessing, observing, teaching, and providing patient-centered care.

-Execute physician orders as required while reporting to the nursing supervisor.

-Maintain medical record documentation.

-Communicate with physicians, social workers or other health advisors as needed

Qualifications:

• Licensed Registered Nurse and reliable method of transportation (vehicle).
• Great communication skills both written and verbal.
• Exceptional time management skills
• Computer skills for basic health care data entry and documentation.
• Ability to work within an interdisciplinary team environment.
• Must be professional, passionate about working with the elderly and dependable.
• Bi-Lingual candidates preferred.
