Job Description

Job Discripition :

1. Understands and adheres to established Agency policies/procedures.
2. Understand and promote the principles of QI
3. Acts as a consultant to other Agency personnel.
4. Participates in the coordination of the Agency’s services with the services of other community resources; uses community resources.
5. Observe confidentiality and safeguard all patient information.
6. Serves as a resource person to employees, patients, physicians, and other allied healthcare providers.
7. Develop a cooperative relationship and communicate effectively and professionally with physicians.
8. Accepts responsibility for regular attendance and punctuality.
9. Immediately report any accident, incident, lost articles, or unusual occurrence to the Center Manager
10. Participates in in-services and/or continuing education programs.
11. Participate in patient care conferences.
12. Maintains contact with community support groups and provides professional expertise as required.
13. Plans/coordinates all social services within the Agency.
14. Documents all patient/family services provided as required by Agency policy.
15. Assists physician and other team members in understanding the significant social and emotional factors related to health problems.
16. Participates in the development of the plan of care.
17. Prepares clinical and progress notes.
18. Works with the family.
19. Participates in discharge planning.
20. Other duties assigned by Center Manager

Qualifications

1. A person who has a master’s degree from a school of social work accredited by the Council on Social Work Education, and has one year of social work experience in a healthcare setting.
2. Two (2) years of appropriate experience in Geriatrics/ Long-Term Care
3. Ability to exercise initiative and independent judgment.
4. Ability to work with individuals to enlist the cooperation of many people to perform/achieve a common goal.
