Position - Registered Nurse (RN)/Home Health
Status/Schedule - Full Time, Monday - Friday, 8a-5p with On-Call rotation
Registered Nurse (RN) Position Summary
- Provide one to one care to patients in their homes.
- Support patient recovery and ability to stay at home.
- Educate patients and families on disease management.
- Case manage and provide clinical oversight to interdisciplinary team.

Registered Nurse (RN) Education and/or Experience
- Graduate of an accredited college/university; Associate or Bachelor's degree from School of Nursing required.
- Two years of experience as a registered nurse in a clinical care setting, home health preferred.
Registered Nurse (RN) Certifications, Licenses and Registrations
- Current, unencumbered state license as a Registered Nurse.
- Current CPR certification.
