What are the responsibilities and job description for the LPN-Clinic position at Ascension?

We Are Hiring

Hours: 7:30am -5:30pm, Monday-Friday, specific days and hours to be determined based upon the needs of practice.

 

LPN experienced with patient care and telephone triage Pediatric experience preferred. Candidate will be responsible for assessing patient's condition/symptoms, giving appropriate clinical direction, education, and recommendation(s). Will respond to incoming calls in a timely, professional manner and triage to support the needs of patients and practice physicians.

What You Will Do

Provide care to patients under the direction of a registered nurse or physician, functioning within scope of license.

  • Contribute to the assessment of patients and assure the well-being of patients.
  • Administer medications and observe patients for adverse reactions to medications or treatments.
  • Perform laboratory tests and therapeutic services.
  • Educate patients/families about disease treatment plan including self care post discharge, holistic health needs, available resources and follow up care.

What You Will Need

Licensure / Certification / Registration:

  • Certified BLS Provider obtained within 6 Months (180 days) of hire date or job transfer date required.
  • Practical/Vocational Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.Valid state license required specific to the state in which the associate works required.

Education:

  • High school diploma or GED required.
