Responsibilities
Coordinate access to health-related services by assisting clients with utilizing available resources.
- Identify potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.
- Provide informal counseling, education, and social support to clients. Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
- Develop and maintain strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
- Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference
Requirements
Licensure / Certification / Registration:
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Drivers License preferred.
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
Patient facing experience preferred.
